1. Sickness certification: see HERE.

2. Flu and Pneumonia Vaccination: please check HERE for information on the Flu vaccine, and HERE for information on the Pneumonia vaccine.

3. Driver’s Licence Medicals:

Please bring the forms to your doctor who will then check your medical history and perform the necessary medical examination.

Forms must be printed on both sides prior to getting a doctor’s signature.

4. Blood tests:

Please check with your doctor if you require blood tests. Do not book directly with the nurse.
