Chronic Disease Management

The structured Chronic Disease Management (CDM) Programme aims to prevent and manage patients with chronic diseases. It helps us identify and manage patients who have been diagnosed with one or more of the following diseases:

1. Type 2 diabetes

2. Asthma

3. Chronic obstructive pulmonary disease (COPD)

4. Cardiovascular disease, including heart failure, heart attack (angina), stroke and irregular heartbeat (atrial fibrillation).

If you have any of the above diseases, or are at high risk of developing one, your doctor will request that you attend once every six months for an examination and/or blood tests in order to prevent progression of the disease.
